'Ondoy' victims can renew passport FREE+expedited!

MANILA, Philippines – People whose passports were lost or damaged because of the recent floods caused by tropical storm Ondoy can apply for a new one free of charge, according to the Department of Foreign Affairs.


Applicants can avail of an “affidavit of loss or damage” free of charge at DFA’s Office of Consular Affairs (OCA), according to foreign affairs undersecretary Rafael Seguis.


Requirements for lost passports include: birth certificate issued by the National Statistics Office (NSO), ID or other supporting documents indicating the date and place of birth of the applicant, three passport size pictures and duly accomplished passport application form (which can be downloaded at www.dfa.gov.ph).

It is also expedited so it would be less than the usual 15 days.

If you lost your passport:
1. Personal appearance
3. Valid ID
4. accomplished Application form
5. (3) passport size photo

If the passport is damaged/torn apart:
1. Personal appearance
2. Valid ID
3. No need for NSO Authenticated Birth Certificate but bring the damaged passport.
4. accomplished Application form
5. (3) passport size photo
